Older adults: function and frailty matter
Chronological age alone does not describe the ability to tolerate a medical intervention.

Look beyond an age cutoff
Two people of the same age may differ in mobility, cognition, nutrition, heart and lung function, and medication burden. A reasonable assessment considers these differences in relation to the proposed procedure. Ask what factors make treatment suitable or unsuitable and which findings would change the plan.
Clarify the goal
A specific goal such as improved walking endurance is easier to evaluate than a broad promise of rejuvenation. Discuss established ways to address that goal and how they compare in evidence, burden, and cost. A biological-age test or wellness score should not substitute for patient-relevant outcomes.
Plan support realistically
Consider transport, stairs, medication management, hearing or language needs, and help after discharge. Bring a trusted person if desired, while preserving the patient’s own decision-making role. If a clinic proposes multiple procedures in one visit, ask whether the combined burden has been assessed rather than evaluating each component separately.
Putting the discussion into practice
When the goal is maintaining independence, ask whether the proposed outcome connects to daily activities such as walking, dressing, or preparing meals. An intervention that changes a laboratory marker without improving function may not meet that goal. Consider whether travel itself creates fatigue or access difficulties. A comprehensive assessment can reveal that supporting established care at home is a more useful next step.
Questions to take to the consultation
Ask: Which personal factors increase the burden of this plan? What practical support will I need? Does the proposed outcome improve a function I value? What alternative might achieve that goal with less disruption? Include a medication review and an honest discussion of travel fatigue. When a package includes several treatments, ask whether the team has considered their combined effects on mobility, recovery, and the ability to manage safely away from home. Include accessibility in the practical assessment: transfers, wheelchair access, bathroom arrangements, and help at airports may matter more than the appearance of the clinic. Ask who can assist without assuming that a medical tourism package includes trained caregiving.
