Ask what support is required

The team can explain the expected caregiver role for the proposed transplant pathway. Discuss attendance at teaching sessions, transport, medicine support, and monitoring. The needs may be more extensive than those of a companion on an ordinary medical trip.

Learn the care plan together

Attend key discussions with the patient’s permission and keep a shared written schedule. Ask for practical demonstrations when a task requires training. Clarify what the caregiver should observe and when to contact the clinical team rather than trying to manage a medical problem alone.

Plan for limits

Consider employment, sleep, other dependants, and the caregiver’s own health. Arrange backup where possible and tell the transplant service if support is uncertain. An honest discussion about capacity is more useful than agreeing to responsibilities that cannot be sustained.

Maintain the patient’s voice

Support should not remove the patient from decision-making. Ask how they prefer information to be shared and when they want private time with clinicians. Caregiver planning works best when practical help, emotional support, and respect for autonomy are discussed explicitly.

A caregiver task map

Divide tasks into those the patient manages, those a caregiver assists with, and those requiring clinical staff. Include appointment scheduling, transport, medicine reminders, record keeping, and household responsibilities. Review the map with the transplant team, because some tasks may require teaching or specific precautions. This makes the workload visible and helps identify where professional or additional family support is needed before everyone becomes exhausted.

Questions about backup support

Ask the service what options exist if the planned caregiver cannot continue. Discuss whether another person can attend training and share the role. Clarify how the patient can contact clinicians directly when the caregiver is absent. A backup plan should be agreed while there is time to organize it, rather than discovered during a crisis or after a caregiver becomes too tired to perform tasks safely.
