Ask for a written schedule
A transplant recipient’s vaccination needs should be reviewed by the transplant team. Ask when the schedule will be discussed and who will coordinate it after discharge. Do not simply resume an old routine or follow a travel-clinic checklist without informing the transplant service.
Bring past records
Collect previous vaccination documents and any relevant infection history. Give copies to both the transplant team and the clinician who will provide care at home. Missing records should be identified early so the team can decide how to proceed.
Coordinate travel advice
If future travel is planned, tell the clinicians well in advance. Some destination recommendations may not suit a person with an altered immune system or current immunosuppressive treatment. Ask the transplant specialist to coordinate with a travel-medicine clinician rather than making independent assumptions.
Keep responsibility clear
Record who will arrange appointments, update the schedule, and document doses. Review the plan if medicines or health status change. Vaccination decisions after transplant are part of long-term clinical follow-up, not a generic wellness add-on.
A vaccination record handover
Create a single record shared with the transplant team and home clinician, noting previous vaccinations and the new plan when it is issued. Ask who will update it after each dose. If care transfers between countries, include generic vaccine names and dates rather than only local brand names. This helps avoid gaps or assumptions when different services use different schedules or record systems.
Questions before any new vaccine
Tell the vaccinating clinician about the transplant, current medicines, and the transplant team’s plan. Ask them to confirm suitability with that team when needed. Do not assume that a vaccine recommended to a healthy travelling companion is suitable for you at the same time. If an appointment is postponed because health or treatment changes, ask how the overall schedule should be adjusted and who will document the revision.
