Seek care first
If symptoms are severe or rapidly worsening, obtain urgent assessment. Reporting systems do not provide emergency treatment. Tell the clinician about the recent cell-based procedure and all medicines used, including products administered at a separate clinic.
Record the timeline
Write down the procedure date, onset of symptoms, changes over time, and care received. Preserve discharge records, invoices, product identifiers, and relevant communications. Describe what happened without assuming that the procedure caused every later symptom; clinical assessment and investigation may be needed to determine the relationship.
Notify the treating institution
Use the clinic or hospital’s clinical contact and ask how the event will be documented and reviewed. Request a copy of the relevant record and a plan for ongoing care. Keep communication factual and retain the replies.
Use the appropriate reporting channel
Ask a treating clinician which regulator or patient-safety channel applies in the country where care occurred. The FDA provides a reporting pathway for relevant events in its jurisdiction; this does not replace Korean processes. When making a report, include enough detail to identify the intervention and avoid sharing unrelated personal information.
A factual event summary
A useful summary states what was administered, when symptoms began, what symptoms occurred, and which clinicians assessed them. Include dates and copies of relevant findings. Avoid adding conclusions that a clinician has not made, such as naming a particular mechanism of injury. Clear factual information supports investigation and can be updated as more evidence becomes available, while a dramatic but imprecise account may be harder to evaluate.
Questions about ongoing care
Ask the evaluating doctor what follow-up is needed and who will coordinate it. Ask the original institution to acknowledge the event and provide relevant preparation records. Keep reporting and treatment pathways separate: filing a complaint or safety report does not ensure that a medical appointment has been arranged. If you need further assessment, make that appointment directly and bring the documentation collected so far.
