Choose a small set of measures
Record symptoms and function connected to the goals agreed before treatment. Include the date, medicines taken, activity level, and any significant event. A short consistent record is usually more useful than a long irregular narrative that is difficult for your doctor to review.
Use comparable observations
Where practical, assess the same task at a similar time of day. Note sleep, rehabilitation, and other treatments that could affect how you feel. Avoid repeatedly testing a painful activity beyond the limits recommended by your clinician just to obtain a score.
Separate observation from explanation
Write that pain increased after a long walk rather than concluding that the cells stopped working. A diary can reveal a pattern but cannot by itself establish the cause. Bring the record to follow-up and ask the clinician to interpret it alongside examination and other findings.
Act on urgent changes
A diary is not a substitute for seeking help. Follow your discharge instructions when a concerning symptom develops, and seek urgent assessment for severe or rapidly worsening problems. Record what happened after obtaining appropriate care, rather than waiting for the next planned appointment to mention it.
A simple diary entry
An entry might record the date, a specific daily task, the symptom rating agreed with your clinician, medicines taken, and an unusual event such as poor sleep or a long journey. Keep the wording factual. If a task could not be completed, record that instead of assigning an optimistic score. Consistency makes a small diary more informative than many photographs or frequent impressions collected without a common method.
Questions at diary review
Ask which pattern matters clinically and whether further examination is needed. If the clinician proposes another procedure, ask how the diary supports that recommendation. Do not let the record become a reason to postpone care for new symptoms. It is a communication tool that complements assessment, and its value comes from helping a clinician understand your experience over time rather than proving success independently.
